V roce 2016 pak v\u00fdzkumn\u00e1 data z Hannoveru potvrdila vysok\u00fd pod\u00edl bimod\u00e1ln\u00edch pacient\u016f mezi u\u017eivateli kochle\u00e1rn\u00edch implant\u00e1t\u016f v\u0161ech v\u011bkov\u00fdch skupin. Mezi roky 2010 a 2015 narostl po\u010det bimod\u00e1ln\u00edch pacient\u016f t\u00e9m\u011b\u0159 o 40 %.<\/p>\n<p>Jedna n\u011bmeck\u00e1 studie poskytla v\u011bdeck\u00fd d\u016fkaz o v\u00fdhod\u00e1ch bimod\u00e1ln\u00edho sly\u0161en\u00ed<a href=\"#_ftn1\" name=\"_ftnref1\">[1]<\/a>: V\u00fdhody bimod\u00e1ln\u00edho sly\u0161en\u00ed z\u00e1vis\u00ed p\u0159ev\u00e1\u017en\u011b na sluchov\u00e9m prahu. Tyto v\u00fdhody jsou n\u00edzk\u00e9 pouze v p\u0159\u00edpad\u011b, \u017ee se sluchov\u00e1 pr\u00e1h na druh\u00e9m uchu bl\u00ed\u017e\u00ed stavu, kdy by bylo vhodn\u00e9 i do tohoto ucha implantovat kochle\u00e1rn\u00ed implant\u00e1t. Pacient\u016fm s lep\u0161\u00edm sluchem na akustick\u00e9 stran\u011b bimod\u00e1ln\u00ed sly\u0161en\u00ed p\u0159ineslo velk\u00fd prosp\u011bch.<\/p>\n<p>Princip bimod\u00e1ln\u00edho sly\u0161en\u00ed vy\u017eaduje, aby bylo dan\u00e9 naslouchadlo kompatibiln\u00ed s dan\u00fdm kochle\u00e1rn\u00edm implant\u00e1tem. \u201eMnoho lid\u00ed si mysl\u00ed, \u017ee tu jde jen o proud dat,\u201c povdzechne si Jennifer Robinson, kter\u00e1 je zku\u0161enou audiolo\u017ekou a projektovou mana\u017eerkou MED-EL. Sama d\u016fkladn\u011b studovala potenci\u00e1l bimod\u00e1ln\u00edho sly\u0161en\u00ed, ale tak\u00e9 jeho pot\u00ed\u017ee a nedostatky. Vysv\u011btluje, \u017ee v tomto probl\u00e9mu je t\u0159eba vz\u00edt v \u00favahu dva aspekty: syst\u00e9m implant\u00e1tu by m\u011bl co nejl\u00e9pe napodobovat p\u0159irozen\u00e9 sly\u0161en\u00ed. Kochle\u00e1rn\u00ed implant\u00e1ty v\u0161ak na rozd\u00edl od nich stimuluj\u00ed sluchov\u00fd nerv p\u0159\u00edmo a napodobuj\u00ed tedy funkci vnit\u0159n\u00edho ucha. \u010c\u00edm vice se jejich funkce bl\u00ed\u017e\u00ed funkci lidsk\u00e9ho ucha, t\u00edm l\u00e9pe m\u016f\u017ee b\u00fdt zaznamenan\u00fd zvukov\u00fd sign\u00e1l skombinovan\u00fd se zvukov\u00fdm sign\u00e1lem z druhostrann\u00e9ho ucha (na tzv. kontralater\u00e1ln\u00ed stran\u011b). Proto by se k\u00f3dov\u00e1n\u00ed zvukov\u00e9 frekvence i \u010dasov\u00e9 k\u00f3dov\u00e1n\u00ed m\u011blo co nejv\u00edce p\u0159ibli\u017eovat princip\u016fm p\u0159irozen\u00e9ho sly\u0161en\u00ed.<a href=\"#_ftn2\" name=\"_ftnref2\">[2]<\/a><\/p>\n<p>K tomu je za pot\u0159eb\u00ed dlouh\u00e1 elektroda, kter\u00e1 m\u016f\u017ee b\u00fdt pln\u011b vlo\u017eena do hlem\u00fd\u017ed\u011b vnit\u0159n\u00edho ucha. Tato elektroda mus\u00ed pokr\u00fdvat celou d\u00e9lku hlem\u00fd\u017ed\u011b, aby byly pat\u0159i\u010dn\u011b zaznamen\u00e1ny jak vysok\u00e9, tak i n\u00edzk\u00e9 t\u00f3ny, stejn\u011b jako v na\u0161em uchu.<a href=\"#_ftn3\" name=\"_ftnref3\">[3]<\/a> V t\u00e9to chv\u00edli je na sv\u011bt\u011b pouze jeden v\u00fdrobce, kter\u00fd nab\u00edz\u00ed implant\u00e1t spl\u0148uj\u00edc\u00ed tento po\u017eadavek:<a href=\"https:\/\/www.medel.com\/\" target=\"_blank\" rel=\"noopener\">MED-EL<\/a>.<\/p>\n<h2>D\u016fle\u017eitost \u010dasov\u00fdch rozd\u00edl\u016f<\/h2>\n<p>Pro p\u0159irozen\u00e9 sly\u0161en\u00ed n\u00edzk\u00fdch t\u00f3nu je d\u016fle\u017eit\u00e9 \u010dasov\u00e9 k\u00f3dov\u00e1n\u00ed zvuku.<a href=\"#_ftn4\" name=\"_ftnref4\">[4]<\/a> Toto k\u00f3dov\u00e1n\u00ed popisuje podrobnou \u010dasovou anal\u00fdzu p\u0159ich\u00e1zej\u00edc\u00edho zvukov\u00e9ho sign\u00e1lu. Tuto p\u0159esnou struktur\u00e1ln\u00ed informaci je dnes schopen zaru\u010dit pouze jeden syst\u00e9m kochle\u00e1rn\u00edch implant\u00e1t\u016f na trhu: MED-EL SYNCHRONY. Jeho jemn\u00e9 struktur\u00e1ln\u00ed k\u00f3dov\u00e1n\u00ed v kombinaci s dlouhou atraumatickou elektrodou, je\u017e jsou oba sou\u010d\u00e1st\u00ed technologie <em>Triformance, <\/em> je schopno replikovat \u010dasov\u00e9 k\u00f3dov\u00e1n\u00ed p\u0159irozen\u00e9ho sly\u0161en\u00ed. Rychl\u00e9 zpracov\u00e1n\u00ed dat v obou syst\u00e9mech zaru\u010d\u00ed, \u017ee nedojde k \u017e\u00e1dn\u00e9mu zpo\u017ed\u011bn\u00ed, a d\u00edky tomu m\u016f\u017ee pacient sly\u0161et z\u00e1rove\u0148 ob\u011bma u\u0161ima.<\/p>\n<p>Dal\u0161\u00ed d\u016fle\u017eit\u00fd \u010dasov\u00fd aspekt sly\u0161en\u00ed je zalo\u017een na mikro-mechanice vnit\u0159n\u00edho ucha. Pohyb proch\u00e1zej\u00edc\u00ed vlny, kter\u00e1 p\u0159en\u00e1\u0161\u00ed zvuk z b\u00e1ze vnit\u0159n\u00edho ucha a\u017e k jeho apexu, zp\u016fsobuje jev, p\u0159i n\u011bm\u017e t\u00f3ny o vysok\u00e9 frekvenci doraz\u00ed ke sv\u00fdm receptor\u016fm o n\u011bkolik milisekund d\u0159\u00edve ne\u017e t\u00f3ny o n\u00edzk\u00e9 frekvenci, kter\u00e9 cestuj\u00ed a\u017e ke \u0161pi\u010dce hlem\u00fd\u017ed\u011b. \u201cJsme jedin\u00fdm v\u00fdrobcem kochle\u00e1rn\u00edch implant\u00e1t\u016f, kter\u00fd p\u0159i konstrukci zohlednil toto \u010dasov\u00e9 zpo\u017ed\u011bn\u00ed,\u201d vysv\u011btluje hrd\u011b Robinson a odkazuje na<a href=\"#_ftn5\" name=\"_ftnref5\">[5]<\/a>n\u011bmeckou srovn\u00e1vac\u00ed studii, kter\u00e1 toto tvrzen\u00ed potvrzuje. \u201cD\u00edky na\u0161\u00ed stimulaci jsme velmi bl\u00edzko p\u0159irozen\u00e9mu sly\u0161en\u00ed.\u201d<\/p>\n<h2>P\u0159izp\u016fsoben\u00ed hlasitosti<\/h2>\n<p>Zp\u016fsob zpracov\u00e1n\u00ed sign\u00e1lu v audio procesorech hraje pro pacienty s binaur\u00e1ln\u00edm sly\u0161en\u00edm st\u011b\u017eejn\u00ed roli. M\u011bly by b\u00fdt pou\u017eity pouze takov\u00e9 charakteristiky a funkce, kter\u00e9 jsou k dispozici jak v naslouchadle, tak i v audio procesoru samotn\u00e9m. Audiologov\u00e9 a implant\u00e1tov\u00ed in\u017een\u00fd\u0159i mus\u00ed tento fakt vz\u00edt v \u00favahu p\u0159i sestavov\u00e1n\u00ed implant\u00e1tu.<\/p>\n<p>Jednou z obzvl\u00e1\u0161t\u011b d\u016fle\u017eit\u00fdch funkc\u00ed je tzv. dynamick\u00e1 komprese. Pln\u011b funk\u010dn\u00ed vnit\u0159n\u00ed ucho je p\u0159ibli\u017en\u011b t\u0159ikr\u00e1t citliv\u011bj\u0161\u00ed na tich\u00e9 zvuky ne\u017e na hlasit\u00e9 zvuky. Zesilovac\u00ed funkce hlem\u00fd\u017ed\u011b pro tich\u00e9 zvuky se u osob se ztr\u00e1tou sluchu postupn\u011b sni\u017euje. V\u00fdrobci naslouchadel se teto jev sna\u017e\u00ed kompenzovat za pomoc\u00ed sv\u00fdch za\u0159\u00edzen\u00ed.<\/p>\n<p>MED-EL je v dne\u0161n\u00ed dob\u011b jedin\u00fdm v\u00fdrobcem, kter\u00fd toto p\u0159irozen\u00e9 zes\u00edlen\u00ed zvuku s pom\u011brem 3:1 (a mo\u017enou variantou 2:1 a\u017e 3,5:1) vyu\u017eil p\u0159i konstrukci sv\u00fdch audio procesor\u016f. Z tohoto d\u016fvodu syst\u00e9my kochle\u00e1rn\u00edch implant\u00e1t\u016f MED-EL perfektn\u011b dopl\u0148uj\u00ed b\u011b\u017en\u00e1 naslouchadla na druhostrann\u00e9m uchu \u2013 bez pot\u0159eby specifick\u00e9ho propojen\u00ed mezi t\u011bmito dv\u011bma za\u0159\u00edzen\u00edmi.<\/p>\n<p>Celkov\u00e1 hlasitost obou za\u0159\u00edzen\u00ed by m\u011bla b\u00fdt p\u0159esn\u011b vylad\u011bna. Studie prok\u00e1zaly, \u017ee naslouchadla by m\u011bla b\u00fdt stejn\u011b hlasit\u00e1 jako syst\u00e9m kochle\u00e1rn\u00edho implant\u00e1tu, nebo jen o n\u011bco m\u00e1lo ti\u0161\u0161\u00ed.<a href=\"#_ftn7\" name=\"_ftnref7\">[7]<\/a><\/p>\n<h2>Pomocn\u00e1 za\u0159\u00edzen\u00ed pro oba syst\u00e9my<\/h2>\n<p>Pomocn\u00e1 sluchov\u00e1 za\u0159\u00edzen\u00ed by m\u011bla b\u00fdt kompatibiln\u00ed s audio procesorem a naslouchadlem. Induk\u010dn\u00ed smy\u010dky jsou vhodn\u00e9 pro naslouchadla, stejn\u011b jako pro audio procesory s induk\u010dn\u00edmi porty. Zaru\u010duj\u00ed pacient\u016fm nez\u00e1vislost na v\u00fdrobci naslouchadla a implant\u00e1tu.<\/p>\n<p>Pro bimod\u00e1ln\u00ed pacienty je k dispozici mnoho univerz\u00e1ln\u00edch pomocn\u00fdch sluchov\u00fdch za\u0159\u00edzen\u00ed a komunika\u010dn\u00edch syst\u00e9m\u016f, jako nap\u0159. ty od spole\u010dnost\u00ed Oticon nebo Phonak \u2013 tyto syst\u00e9my p\u0159en\u00e1\u0161\u00ed audio z\u00e1znamy prost\u0159ednictv\u00edm Bluetooth. Syst\u00e9m Roger od spole\u010dnosti Phonak nab\u00edz\u00ed indukci, stejn\u011b jako p\u0159\u00edm\u00fd vstup do audio procesor\u016f a naslouchadel.<\/p>\n<h2>U\u010d\u00edme se od Popelky<\/h2>\n<p>D\u00edky poh\u00e1dce o Popelce jsme se nau\u010dili jednu podstatnou v\u011bd. Pokud se chcete vl\u00e9zt do men\u0161\u00edch bot, nezkou\u0161ejte si u\u0159ezat palce, ani tak v\u00e1m boty nepadnou.
